Skilled Nursing Care Our registered nurses will assess a patient's needs and provide intermittent nursing visits for the treatment of illness or injury. Medical Social Work Our goal is to help resolve social and emotional problems that are an impediment to the recovery of the patient. Our social work team offers counseling to assist in the adjustment to an illness. We'll also help patients to find valuable community resources that can assist them in becoming more independent.

Certified Home Health Aide Our RN-supervised aides provide day-to-day services to improve the quality of life of patients. Assistance with bathing, grooming, feeding, ambulation, housekeeping and meal preparation are all services that can be performed by our Certified Home Health Aides.

Congestive Heart Failure Program We oversee and implement the treatment for cardiac disease with a team approach. Specially trained registered nurses, physical therapists, occupational therapists, social workers, nutritionists and home health aides take part in an integrated program designed to meet the needs of home care patients.

Home Infusion Therapy When treatment calls for infusion therapy, our home nursing services can make it easier on the patient. We provide nursing, pharmaceuticals, supplies and equipment to meet an individualized treatment plan ordered by a physician and approved by the patient's health plan. We also teach both the patient and caregiver about treatment to help them function independently during the process. Staffed by nurses who specialize in home infusion, our program helps improve the quality of life for the patient and lowers treatment costs.
Home Medical Equipment and Supplies By Arrangement
Whether you need a wheelchair, hospital bed, bathroom safety aids or more custom-designed health equipment, we can find the right equipment and/or supplies for virtually any patient need. We can assist in selection, provide instructions and set-up any equipment you choose. Along with medical equipment, we also provide clinical services. You can call on us to provide everything from oxygen systems and aerosol therapy for patients with breathing problems or diseases to custom orthotics for patients who need support devices to aid mobility. Hospice Care We provide a coordinated program of care - including physical, psychosocial and spiritual support - for patients with terminal illnesses and their families. A compassionate interdisciplinary team provides nursing, counseling and social, pastoral and personal care. Counseling and emotional support are provided to both patient and family. Pharmaceuticals and durable medical equipment and supplies that are part of the plan of care are also provided.

Maternal-Child Care Program When a new mother and baby arrive home from the hospital, our specially certified registered nurses can make life a little more reassuring. They can instruct new mothers on breastfeeding, formula preparation, and weighing and bathing the baby. And the emotional support our nurses offer can make the transition from hospital to home life a smooth one. High-risk obstetrical services are also available for a wide range of conditions and are a welcome alternative to hospitalization for expectant mothers who either need or want medical supervision after childbirth.

Mental Health Program Patients with mental health care needs can be cared for at home by our certified registered nurses. Every member of our mental health nursing staff understands the need for sensitivity in caring for mental health patients and has at least two years of inpatient psychiatric nursing experience.

Enterostomal Therapy Our skilled nurses have been specially trained to provide individualized home treatment for all types of wounds and ostomies. Nurses are current in the most up-to-date wound treatments and ostomy appliances, so that even the most difficult cases can be handled at home.
