| When your surgery or procedure has been scheduled, please contact Pre-Admission Testing for an appointment (610) 270-2220. It is very important that you are seen well before your procedure date. This will prevent delays on the day of your procedure. Even if no tests are ordered, a visit to the Pre-Admission Testing Center is necessary. This visit enables us to check all paperwork, complete registration forms and review your insurance plan for approval by the provider. Testing, if ordered, anesthesia consultation, and a history and physical will be completed at this time.
Steps to Prepare for your Pre-Admission (PAT) Appointment
1) Call for your PAT appointment as soon as you know your procedure date
2) Call your family doctor and inform him/her of your upcoming procedure
3) Make a complete list of all medications you are taking with dosages
4) Bring to PAT:
- Pre-Admission Testing Orders
- Surgical/Procedure Consent Form, completed
- Test results completed at other physician offices
- List of current medications and dosages
- All applicable insurance forms:
